◆ Step 1: Create an Account and Complete the Log-in Process. THIS IS WHERE YOU WILL SELECT ALL SPORTS THE ATHLETE WILL TRY OUT FOR DURING THE ENTIRE SCHOOL YEAR.
◆ Step 2: Fill Out All Sections and Online Forms.
◆ Step 3: Complete the Online Signature Agreements.
◆ Step 4: Print and sign the Confirmation page. Parent and Student must sign. You can print from the site, from the email that is sent, or take a screenshot and sign on your phone.
◆ Step 5: Email the signed Confirmation and a CURRENT Physical with a Physician’s stamp on the form (MD, or DO only) to aholzheimer@guhsd.netThis process must be done to participate in any sport. Physical is valid for one calendar year.

INSURANCE:
● All athletes MUST have current health insurance to try out and participate. Do not leave the Insurance Company and policy number blank on the athletic clearance process. Athletes will not be cleared if this information is not provided.

PHYSICALS:
✅Physical Form:Download the form
Step 1: The parent completes and signs the top section of the form prior to the doctor's visit.
Step 2: Doctor (MD or DO only) must stamp & sign the completed physical form.
Step 3: Email the signed Confirmation and a current physical with a Physician’s stamp on the form (MD, or DO only) to aholzheimer@guhsd.net
Physicals are required annually and must be valid for the entire season of the sport.
● Physicals are good for one year from the date of the exam – not the date the physician completed the form. Physicians must be licensed by the State of California.
