PHYSICAL/INSURANCE
A physical examination is required before a student can try out, practice, or compete.
A Physician's stamp is required on the physical form. Physicals are good for one calendar year from date of physical. Medical/accidental death and bodily injury insurance is required to participate in a school sport (including cheer). The insurance can be provided by the parents' personal insurance coverage or, if your student needs insurance, the district offers Student Accident Insurance which meets the requirements for athletic participation. Forms are available in the Finance Offices. In either case, the parent is responsible for providing insurance.
ATHLETIC CLEARANCE
Students and parents/guardians must complete the Athletic Clearance Packet prior to playing a sport.
In the system, you will be required to clear for EACH SPORT INDIVIDUALLY. Physical forms are in the packet. Physicals are good for the current school year.
Athletic Clearance Packets are available online. The packet must be 100% complete before you can be cleared to participate in sports.
